Report Finds 'Missed Chances' To Save Daniel Pelka

A report on the death of four-year-old Daniel Pelka has found that "no professional tried sufficiently hard enough" as the child suffered "terrifying and dreadful" abuse by his mother and step-father.

Daniel died in March 2012 after suffering months of being beaten and starved. The report found that chances to help Daniel were missed, saying that the child was "invisible" at times and adding that "critical lessons" must be "translated into action".

Daniel's mother, Magdelena Luczak (27) and step-father, Mariusz Krezolek (34), were found guilty of murder, and sentenced to minimum terms of 30 years.

The report found that details of the level of abuse Daniel suffered were "completely unknown" to professionals involved.

Key findings of the report include:

• 26 police call outs to Daniel's home, many involving domestic violence and alcohol abuse

• Agencies accepted Magdelena Luczak's explanations for Daniel's condition

• His school and health workers made no "committed attempts" to address Daniel's "health and behavioural issues".

It is understood that to date, no-one has been disciplined as a result of Daniel's death.

The British bomb disposal expert killed in Afghanistan by a roadside bomb on Monday has been named as Captain Daniel Shepherd. Capt Shepherd was serving with the Joint Force Explosive Ordnance Disposal Group when he died following an explosion in Nad-e-Ali district in Helmand Province, Southern Afghanistan.

Danny La Rue Dies, Aged 81
Entertainer Danny La Rue has died at his home in Kent, following a short illness, his spokesperson has confirmed. The 81-year-old - best known for his legendary female impersonation act - died after suffering from cancer. His spokeswoman said: "Danny died peacefully in his sleep just before midnight last night after a short illness.

Missing 13-Year-Old Found 'Safe And Well'
Thirteen-year-old Daniel Pena-Sanchez has been found "safe and well", police have reported. The Catford teenager was reported missing on Tuesday, 11 August. Police said that Daniel was found after a member of the public recognised him from their social media campaign. The Metropolitan Police thanked the public for their help.
